Geriatric Care Management and Care Coordination

Geriatric Care Management and Care Coordination by our registered nurse focuses on the functionality of the individual. After a thorough assessment of safety, physical, medical, pharmacological, psycho-social, financial and legal issues, a care plan is developed to help assist with current and future needs as well as obtaining the resources necessary to remain in ones own home.

Care management takes the worry out of the twenty-four hour a day dilemma of caring for elderly family members.

Our RN care manager interacts with several professionals involved in your loved ones cares; their physician to insure they are receiving proper medical care and prescriptions, their pharmacist to insure the synergy or compatibility of their medications and other rehabilitation specialists to assure the attainment of their highest level of functionality. Oftentimes elderly individuals do better at home than in a facility and our care manager will orchestrate and aid in their transition and recovery.

Medical care management relieves the stressors that affect families caring for an elderly parent or relative. Our RN arranges for and often accompanies clients to their doctors appointments, sets up and monitors medication regimes, intervenes in times of medical crisis's, procures outside community resources, medical equipment and supplies and maintains contact with family members to keep them abreast of their loved ones status and changes in condition.
